Why Does My Knee Still Hurt Years After Knee Replacement?

A total knee replacement (TKR), also known as total knee arthroplasty (TKA) can reduce severe arthritis pain and restore mobility, but it does not guarantee that all future function and movement will be painless. After total knee replacement, imaging will likely say the implant looks fine. Despite this, your pain lingers and may even feel entirely different from your original arthritis that led you towards TKR to begin with. The pain source may be mechanical, soft-tissue, nerve-related or referred pain from another area. Regardless, further evaluation from an orthopedic specialist is required to mitigate post-operative damage.

More than 790,000 total knee replacements are performed each year in the United States, and that number is expected to keep rising as the population ages. Although many patients do see significant long-term pain reduction, research suggests approximately 20% experience chronic pain after TKR. Persistent knee pain deserves a careful evaluation, particularly when it disrupts sleep, walking or daily activity.

Is It Normal to Have Pain Years After Knee Replacement?

Mild soreness can persist after knee replacement, but new or worsening pain years after TKR should not be dismissed. An evaluation can determine whether your symptoms stem from the implant, surrounding tissues, nerves, the hip or the spine.

Pain that never resolved after knee replacement surgery may have a different cause than fresh pain that begins after months or years of post-surgical function. Your orthopedic specialist will consider when the pain started, it’s location and whether you notice swelling, instability, or neurological symptoms.

Red Flags Requiring Immediate Care

Seek prompt medical care for symptoms that may indicate infection, a blood clot or structural failure. Call 911 for sudden shortness of breath or chest pain.

  • Possible infection: Redness or swelling around the incision, worsening pain or stiffness, or feeling systemically unwell. It is crucial to note that joint-replacement infection can develop months or even years after surgery.
  • Possible deep vein thrombosis (DVT): new calf or leg pain unrelated to the incision, tenderness, redness or increasing swelling of the calf, ankle or foot.
  • Possible structural failure or fracture: a sudden pop followed by severe pain, visible deformity or an inability to bear weight on the replaced knee.

How Long Should Pain Last After Knee Replacement?

Every recovery from a knee operation is unique, and pain should generally trend downward as strength, motion and function trend upward. The following guide is not a substitute for the timeline provided by your surgeon.

Timeframe What’s Normal When to Contact Your Doctor
Month 3 Mild stiffness, swelling after activity. Expect discomfort that gradually improves as strength and range of motion return. Pain is not improving or worsening, function and mobility is declining, or severe nighttime pain is increasing.
6–8 Months Most patients have substantially less pain and better function. Expect kneeling discomfort, numbness near the scar or occasional soreness. Pain remains moderate to severe, limits walking or sleep, or has plateaued despite appropriate rehabilitation.
1 Year Recovery is generally completed, and patient has transitioned to joint maintenance. Minor intermittent soreness can occur, but the knee should be more functional than before surgery. Persistent or recurring pain affects daily life, or new clicking, grinding, catching, or swelling develops.
2–5+ Years A well-functioning replacement may provide many years of relief. Temporary soreness after unusually strenuous activity can still occur. Any new or worsening pain, especially pain with weight-bearing, instability, swelling should be assessed.

 

What Causes Pain Years After Knee Replacement?

Pain years after knee replacement may come from implant loosening, scar tissue accumulation, muscle imbalance, nerve irritation, or pain referred from the hip, spine or sacroiliac joint.

Because several problems can produce similar symptoms, treatment should follow a comprehensive diagnosis, not the location of pain alone.

  • Mechanical or Implant-Related Causes: Mechanical knee pain after TKR often worsens with physical activity, walking or weight-bearing. You may notice an aching feeling, swelling, instability, giving way, clicking, grinding or catching.
  • Soft Tissue, Scar Tissue and Muscle-Related Causes: The knee replacement may be stable while surrounding structures remain painful. Arthrofibrosis, also known as excessive scar tissue, can restrict motion and create stiffness. Muscle weakness or imbalance, which is common as your knee sits immobile for weeks on end post-surgery, can change how weight is loaded onto the joint. This can lead to compensating movement patterns, overuse and irritation in the post-surgical knee.
  • Nerve-Related (Neuropathic) Pain: Burning, tingling, pins and needles, hypersensitivity, or sharp and shooting pain suggests a neuropathic component. Small nerves can be irritated or injured during knee surgery, and postoperative scar tissue may contribute to nerve entrapment.
  • Pain Referred from the Hip or Spine: Not all knee pain after TKA originates in the knee. Hip arthritis, lumbar spine problems, nerve-root irritation and sacroiliac (SI) joint dysfunction can refer pain toward the thigh or knee. A thorough evaluation looks beyond the implant and compares knee findings with hip motion, spinal symptoms, strength, and gait.

How We Determine What’s Causing Your Knee Pain

At Performance Ortho, evaluation begins with your surgical history and the timing, quality and location of your symptoms. A physical examination can assess swelling, range of motion, ligament stability, gait and areas of nerve sensitivity. Your clinician may also examine the hip and spine.

“Pain after a knee replacement does not always mean the implant has failed. Our first priority is to identify where the pain is actually coming from, whether that is the implant, surrounding soft tissue, a nerve or even the hip or spine, so the treatment plan addresses the right problem.”

X-rays can show implant position, alignment, fractures, bone changes and signs that may suggest loosening or wear. Depending on the findings, additional imaging may be needed to investigate structural concerns. No single test answers every case; the findings must be interpreted together.

How Is Persistent Pain After Knee Replacement Treated?

Treatment for chronic knee pain after arthroplasty depends on the source. The goal is to address the identified pain generator while protecting the replaced joint. Revision knee surgery is appropriate for some confirmed implant problems, but persistent pain does not automatically mean another operation is necessary.

Non-Surgical Treatments after TKR

Physical therapy may improve strength, mobility, balance, gait and movement mechanics. Activity modification can reduce aggravation while keeping you active, and a brace may help selected instability or alignment concerns.

When hip, spine or movement dysfunction contributes to symptoms, a coordinated plan may include chiropractic care or other conservative musculoskeletal treatment when clinically appropriate.

Minimally Invasive Interventions

If diagnostic testing identifies a nerve-related source, an image-guided nerve block may temporarily numb targeted sensory nerves and help predict whether genicular nerve radiofrequency ablation (RFA) could provide longer-lasting relief.

Peripheral nerve stimulation (PNS) may be considered for selected chronic neuropathic pain when conservative care has not provided adequate relief.

Corticosteroid or hyaluronic acid injections are commonly used for arthritis in a natural knee, but they do not treat replaced joints. Because injections into a prosthetic knee may carry infection risk, any injection must be selected and performed only after the implant and pain source have been carefully evaluated.

Regenerative Medicine Treatments

Orthobiologics such as platelet-rich plasma (PRP) or stem cell therapy cannot regrow a removed knee joint or repair a loose, infected or failed implant. They may be discussed only when evaluation identifies an appropriate surrounding soft-tissue or separate arthritic target. Candidacy, evidence, risks and realistic expectations should be reviewed individually.

“Regenerative medicine is not a replacement for a failed procedure, and it should never be presented as one. Its potential role is much more specific: helping carefully selected patients address an appropriate soft-tissue or adjacent joint problem after we have ruled out implant-related complications.”

Frequently Asked Questions

Knee pain may feel worse at night because daytime activity can increase inflammation and swelling, while inactivity can contribute to stiffness. A sleeping position that places pressure on the knee may also make discomfort more noticeable.

Treatment starts by medically confirming whether the pain is neuropathic. Physical therapy and clinician-directed medication may help in the short-term. Selected patients may benefit from a diagnostic nerve block, radiofrequency ablation or peripheral nerve stimulation. Burning, tingling, shooting pain or hypersensitivity should be assessed before choosing treatment.

Possible warning signs include increasing pain with weight-bearing, instability, recurrent swelling, declining range of motion, clicking or catching, and a sudden loss of function. Infection can also cause worsening pain, redness, swelling or drainage. Examination, X-rays and sometimes laboratory or joint-fluid testing are needed to determine whether an implant has failed.

Contact a clinician when pain is not steadily improving, lasts beyond the expected recovery period, returns after months of proper function, or limits sleep, walking or everyday activity. Seek urgent care for drainage, rapidly increasing redness or swelling, sudden inability to bear weight, calf swelling, chest pain or shortness of breath.
